Our ongoing dialogue with stakeholders is critical to the success of the CCI and we look forward to continuing our work together to improve health care for Californians. We see many LA Care patients in our ED and struggle to identify the PCP to close the loop on the encounter. Two of our top 10 highest utilizers are LA Care patients without a PCP listed. When we call for verify insurance, we can obtain the name but the addresses to route the Dc documents to are not available on line. How cam we connect this population back to their assigned PCP?
