San Bernardino County
9/8 – Providers and administrative staff from Molina Healthcare attended a CCI presentation, which included an overview of Cal MediConnect and MLTSS. The presentation highlighted the Cal MediConnect care coordinator’s role in the health plan, along with information about the plan options, benefits, and enrollment process. Each attendee received a physician toolkit which includes in-depth information about the CCI.
Orange County
9/9 – More than 400 Korean American Seniors gathered to celebrate the Korean Thanksgiving Fall Festival “Chu-Seok” at the Garden Grove Community Center. Outreach coordinators shared OneCare Connect materials with seniors and key stakeholders in Orange County’s Korean community. Many attendees visited the CCI information table and learned about the benefits of OneCare Connect, care coordination, continuity of care, and plan eligibility.
Santa Clara County
9/29 – At the 24th Annual Senior Wellness and Resource Fair, an outreach coordinator provided a CCI workshop to an audience of seniors, providers, Santa Clara County staff, and community organizations. The participants appreciated learning about the plan’s benefits, especially care coordination.
